Our challenges and themes are developed with our partner organisations, through intelligence gathering about needs and demands in the South West.
In response to the COVID-19 pandemic, we have shifted our focus to providing intensive support to innovators and innovation that supports the aims of the South West Academic Health Science Network (SW AHSN)’s three COVID-19 programmes: improving patient safety, supporting digital-first pathways, and addressing health inequalities.
The South West is a great place for innovation to flourish, from the latest digital solutions supported through our Digital Health Accelerator, to social entrepreneurs helping to 'Transform Ageing'.
Rehabilitation EnAblement in CHronic Heart Failure (REACH-HF)
REACH-HF (Rehabilitation EnAblement in CHronic Heart Failure) is a facilitated evidence based cardiac rehabilitation (CR) and self-management programme for use at home. It comprises the ‘heart failure manual’, a relaxation CD, a choice of exercise (walking programme or a chair-based DVD) a 'progress tracker' for patients, and a ‘family and friends resource’ for caregivers.
Life QI's journey started when an early partnership was formed in 2015 with the South West of England Academic Health Science Network, in England with an aim to deliver an online platform that assisted frontline health and social care staff in running quality improvement (QI) projects. Levels of improvement work varied across a range of organisations with limited accessibility to the data. Staff became increasingly frustrated with using multiple, clunky software applications to record their quality improvement and patient safety work which identified a clear gap in the market for an all-in-one solution.
